INITIAL ASSESSMENT

• With approval, client/family contact within 48 hours of referral
• Submit cost effective treatment and assessment plan
• Meet claimant and family and request information from primary physician
• Identify impairment and disability
• Deliver comprehensive report with specific rehabilitation recommendations

FOLLOW-UP ACTIVITY

• Client meetings as necessary to monitor status and progress
• Initiate referrals to experienced community treatment providers
• Supervise and direct rehabilitation toward defined goals
• Monitor rehabilitation and care costs
• Chair virtual or in-person team meetings to ensure consistent approach in attaining goals
• Client and family education
• Concise follow-up reporting
